What do patients actually enter?

Clinics choose the questions. The default form includes a recognition prompt (so returning visits can be grouped without collecting a name), age range, peptide or protocol, dosage, how long they have been on it, how they are feeling, and optional notes. Clinics can add, remove, or change those questions in settings.

How does the recognition prompt work?

Patients pick a prompt and enter an answer only they would know. Valaria stores a hash of that answer, not the name or contact info, so later submissions can be grouped without collecting PHI.

Does Valaria collect PHI?

The patient form is designed not to collect names, dates of birth, phone numbers, emails, or other direct identifiers. Clinics should not put identifying information in optional notes. See the Privacy Policy for details.
